If you're a Medicaid member, all you need to do is let your insurance provider know you'd like to have CHECK be the Integrated Health Home for you and your family members.  That's it!  You're allowed to change your Integrated Health Home choice monthly, so you can start this process at any time!  CHECK is currently focusing its services in Cook County Illinois, but we hope to branch out soon!  Click your provider below to be directed to their member page where you can tell them you want CHECK as your Integrated Health Home!
